Currently it looks like issue has been contained as it couldn’t be recreated and now Vista is reaching out to internal teams.

Incident Updated: Teams are having a hard time re-creating the original 404 seen by Vista PBM and reported by some customers. Currently issue seems contained, but teams are continuing to try to recreate.
